Registration


Online Registration is required by all Players, Coaches and Club Staff prior to Tryouts!!!


Required documents for new Club Members:

1)  A copy of your child's Birth Certificate to Club Cochise, and a copy mailed to AZ Region with payment.
2)  Bring the original Birth Certificate, it must be verified by a Board Member.
3)  A complete Club Registration Packet.

 

Required documents for returning Club Members:

1)  A complete Club Registration Packet.

 

All players are required to submit a complete Club Registration Packet consisting of:

1) USAV Confirmation Form; completed during online registration process.
2) Medical Release; 2 copies, completed and signed by both the Player, and a Parent.
3) General Conduct Guidelines / Disciplinary Actions; signed by both the Player, and a Parent.
4) Registration and Payment Information (your Contract with CCV); signed by the Player and a Parent.
5) Uniform Info Sheet; completed with sizes, whether a sweatshirt is being purchased (additional cost), uniform number requested, etc.
6) Initial Registration Fee of $150. $50 mailed to AZ Region as part of online registration, $100 to CCV. Full payment or remaining balance may be paid at anytime prior to Regionals.
7) The Payment Coupons for this year will be made available once final Fees Schedule is determined. It is not required to turn in coupons; these are for members use.

Player Registration


Club Cochise currently has five types of Registration Packets for Players:

Travel Team - For Teams Participating in tournaments outside normal region tournaments. Will normally be 16 and 18's teams.
Standard - for all teams based in Sierra Vista.
Partner - for teams based with a partner organization.
Instructional - for non-Club Division teams; designed to teach volleyball skills to girls and boys.

Coach Registration


Required documents & fees for Coaches:

1) USAV Confirmation Form; completed during online registration by the Coach.
2) Coaches Code of Ethics; complete both pages and sign.
3) Consent and Waiver Release Form (Background Check); complete and sign (required every 2 years).
4) USAV fee; $50.
5) Background Check fee; $20 (every 2 years)
6) Coach recertification fee; varies with certification level; Impact- $25 - $100

7) Training completion is required prior to the first tournament, and preferably prior to tryouts.

Chaperone Registration


Required documents & fees for Chaperone:

1) USAV Confirmation Form; completed during online registration by the Chaperone.
2) Chaperone Responsibility Form; complete and sign.
3) Consent and Waiver Release Form (Background Check); complete and sign (required every 2 years).
4) USAV fee; $50.
5) Background Check fee; $20 (every 2 years)

Team Rep Registration


Required documents & fees for Team Rep (any staff or board member not a Coach):

1) USAV Confirmation Form; completed during online registration by the Team Rep.
2) Consent and Waiver Release Form (Background Check); complete and sign (required every 2 years).
3) USAV fee; $50 (paid by CCV)
4) Background Check fee; $20 (every 2 years, paid by CCV)
