Registration
Required documents for new Club Members:
1) A copy of your child's Birth Certificate.
2) Bring the original Birth Certificate, it must be verified by a Board Member.
3) A complete Registration Packet.
Required documents for returning Club Members:
1) A complete Registration Packet.
All players are required to submit a complete Registration Packet consisting of:
1) USAV Registration Form; completed and signed on both pages by the Player, and a Parent.
2) Medical Release; completed and signed by both the Player, and a Parent.
3) General Conduct Guidelines / Disciplinary Actions; signed by both the Player, and a Parent.
4) Registration and Payment Information (your Contract with CCV); signed by the Player and a Parent.
5) Uniform Info Sheet; completed with sizes, whether a sweatshirt is being purchased (additional cost), uniform number requested, etc.
6) Initial Registration Fee of $150. Full payment or remaining balance may be paid at anytime.
7) The Payment Coupons for this year will be made available once final Fees Schedule is determined. It is not required to turn in coupons.
