Below you can find links to all the USAV Registration Forms, Medical Release Forms, and Club Cochise Forms that need to be filled out to join Club Cochise Volleyball. Interested players should also download the Player/Parent Handbook, or get one at registration.

     Pay particular attention to which forms require signatures from both the player and a parent, sometimes on both the front and back of a form.
     A copy of the birth certificate is required for new Club members. The original must be verified by a board member. Bring both a copy and the original to tryouts or a meeting.
     Select the appropriate Registration Packet to download or pick-up at the registration meeting.
